How to logde a Claim
It is a Wozani Ma-Afrika Financial Services policy that claims are paid within 48 hours of receiving all necessary documentation. This would apply to benefits such as the immediate cover plan (accident cover) and partial cover plan received during a waiting trial.
Following correct procedure goes a long way towards avoiding problems that arise at claim time.
Documentation to be submitted when lodging a claim:
Certified copies of;
-
BI1663
-
Death Certificate
-
ID document of the deceased
-
Burial Order from the funeral parlour
-
Confirmation from the funeral parlour
-
Or Police Report in case of unnatural death
All the above documents have to be certified by the commissioner of oaths.
**There might be a need to supply further supporting documentation in respect of:
Spouse’s surnames being different (Affidavits or marriage certificate),
Children’s surnames differing with the policyholder’s (Affidavits or birth Certificate),
Proof of schooling if the deceased is a child above 21 not yet 26 and still a student, at a recognized institution of learning.
A police report for all unnatural or accidental deaths.
Proof of Payment
If you have paid by cash into our bank account, we would need receipts of the past 12 months from the month of death going backwards. If you have paid by bank debit order, we would require bank statements reflecting the last three deductions and if you have paid by stop order, we would require the last three salary advices.
As a claim has to be accurately assessed, it must be emphasized that any documentation requested is required solely for authorization with our underwriters, and not to prejudice the client.
